STUDENTS ARE NOT ALLOWED TO CARRY MEDICATIONS ON CAMPUS!
For the safety our students, it is against the rules for them to bring any medications to school, even to just come and "drop it off" at the office.
We know there are times that medications cannot be given at home or that a child will not feel well at school and want an over-the-counter pain reliever, etc. We understand your time is valuable and coming to the school is not always easy in your busy schedule. We can assist with this by keeping medications for your student in a secure location in our clinic by following these procedures:
A parent/guardian needs to bring the amount of medication the student will need while in school to the clinic in the original prescription bottle.
A medication administration form will be completed in the clinic by the parent/guardian.
A parent/guardian needs to bring the new, unopened bottle of medication to the clinic ALONG with a prescription from the doctor for the medication.
Prescriptions can also be faxed to our office, see fax number above.
The medication must match the prescription.
The parent needs to complete a medication administration form in the nurse's office.
INHALERS AND EPI-PENS-
Students that have a prescription for an inhaler or an epi-pen needs to have a action plan completed by their doctor yearly.
The forms are located in the nurse's office in room 1005.
Due to the emergency nature of these 2 medications, I highly recommend you speak with you doctor about obtaining a carry on person order for your child (these are the ONLY medications allowed to be carried on person, but do require a dr. order to do so).
All medications must be retrieved by a parent/guardian at the end of the school year. Medications cannot be held over the summer.
Medications that are not picked up will be destroyed before we dismiss for the summer.
Scoliosis screening is required on all new 9th graders and will be done in October.
Immunization must be up-to-date.
If you have any immunization questions, please contact Ms. Cole at email@example.com.
